Depressie en LSD

Lysergic acid diethylamide (LSD) is een geschikt geneesmiddel bij aanhoudende angst, stress, alcoholisme en depressie. Tot aan 2024 is LSD aan 567 patiënten voorgeschreven. De overgrote meerderheid van de patiënten ervaren significante en positieve korte termijn veranderingen [ 1].

LSD is middel dat bij een lage dosis al werkt. Een doses tussen 0,5 en 2 mcg/kg (100-150 mcg per dosis). De halfwaardetijd (de tijd die nodig is voor de helft van een stof in het lichaam om af te breken ervan is ongeveer 3 uur, variërend tussen 2 en 5 uur. De geestopbeurende effecten duren tot 12 uur lang. Dit is mede afhankelijk van de dosis, tolerantie, gewicht en leeftijd van de persoon [ 2, 3, 4, 5, 6, 7]. 10 microgram LSD wordt gebruikt om optimaal te kunnen functioneren [ 8, 9, 10]. LSD veroorzaakt geen lichamelijke afhankelijkheid met ontwenningsverschijnselen [ 11, 12, 13 ]. Uit medisch wetenschappelijk onderzoek blijkt dat LSD nauwelijks schadelijk is voor het lichaam, zelfs in een hoge dosis [ 14, 15 ]. LSD behoort daarmee tot de categorie van meest veilige geneesmiddelen [ 16, 17, 18, 19, 20, 21, 22, 23, 24, 25, 26, 27, 28]. Bij veelvuldig of langdurig gebruik kan tolerantie ontstaan, en na een enkele dosis herstelt emotionele, fysieke en mentale stabiliteit snel [ 37, 38, 39, 40].

LSD werkt via serotonine-receptoren. De kristalstructuur van LSD in vergelijk met de humane serotonine-receptor 5-HT2B. Het vergelijk laat een conformationele herschikkingen om LSD te accommoderen zien. Hetgeen een structurele verklaring biedt voor de conformationele selectiviteit van het belangrijkste diethylamide-gedeelte van LSD. LSD scheidt buitengewoon langzaam af van zowel 5-HT2BR als 5-HT2AR - een belangrijk doelwit voor de activatie van de geest. Moleculaire dynamica (MD) simulaties suggereren dat de trage bindingskinetiek van LSD te wijten kan zijn aan een "deksel" gevormd door extracellulaire lus 2 (EL2) bij de ingang van de bindingszak. Een mutatie die voorspeld wordt om de mobiliteit van dit deksel te vergroten, versnelt aanzienlijk de bindingskinetiek van LSD en dempt selectief LSD-gemedieerde werving van β-arrestin2. Deze studie onthult een onverwachte bindingsmodus van LSD, belicht belangrijke kenmerken van zijn kinetiek, stereochemie en signalering, en biedt een moleculaire verklaring voor de werking van LSD op menselijke serotonine-receptoren [ 29].
